Brighton & Hove Transitions Strategy Delivery Plan – Draft 1 –
February 2025:
1. Getting The Numbers Right: The increasing demands and challenges of CYP requiring a Transition Plan across the five care pathways in the care and health system in the city. |
Themes:
· Making sure there is good data on all YP going through transition pathways. · Capture good data equally across all communities. · SEND has good data – Education data dashboard refreshed twice a year and is clear. Health and Social Care data not so clear. Different areas of the NHS hold different data. · Care leavers data are not exact for CYP that may need ASC support · Different systems exist for collecting data across different pathways and access is not open to all pathway practitioners · Agree a way for collecting data across all 5 pathways in the strategy and track the journey of YP into adulthood in a way that gives advanced information for planning of services
Draft Actions: 1. Collate accurate data on CYP from years 9 – 13 (14 years old – 18 years old) 2. Collate the data across the five care pathways 3. Convene a multiagency data task and finish group across Education; Social Care; Health incl. complex physical Health and mental health 4. Use the data to plan and start to predict future needs and demand, and agree on the volume

Agreed way forward: 1. Ask Daryl Perilli (Performance Manager, Commissioning and Communities) and his team to pull together data on a. CYP, using year 9/ age 14 as a base line to help predict future Transition to Adulthood planning who might then progress to needing adult services support later. b. CYP with EHCP, who have social care needs, children in care (who will be care leavers) and those with mental health needs. c. the number of young people who are with children’s social care when they turn 18 and 24 for those open to SCDS (Specialist Community Disability Services) d. the number of 14-year-olds with an EHCP not currently open to social care. 2. Ask Ray Woods to put together Sussex Partnership Foundation Trust data on a. CYP under S117 care plans and other children open to CAMHS. 3. Ask Alec Bennett (Performance Lead, Commissioning & Partnerships) to a. identify how many of YP with EHCP go on to receive a service from Adults Social Care. 4. Convene data and performance group to include all the above on a task and finish action

2. Assessment, Planning & Commissioning: The annual review process is led by schools and colleges; all services need to engage in the reviews. Children’s Social Care & Adult Social Care Services to lead on the coordination of this and the commissioning priorities that emerge from the review process. |

Draft Actions: 1. Use the year 9 SEN Review as the formal start of the Transitions process and/or year 9 review 2. Use the data for proactive Transitions Tracking – across education, social care and health 3. Convene termly Transitions Panels to identify cohorts of YP who will need support in adulthood 4. Ensure joined – up approaches to commissioning services – Commissioners to participate in Transitions Panels

3. Information Advice & Guidance (IAG): A consistent IAG offer across the five care pathways, using existing resources and developing a clear plan for monitoring and updating the information. |

Lots of different platforms existing · Work on refreshing Local Offer website · Amaze set up CYP website to be launched January 2025 information on YP transitioning to adulthood. Enhances information on Local Offer website · Integrated Care Board (ICB) launched large pan Sussex CYP mental health & ND communications programme – exploring and improving all NHS communications · Working group looking at ensuring there is clear information on Local Offer for every age group · Careers advice website
Draft Actions: 1. Improve quality and access for YP and parents/carers to Transitions information 2. Redevelopment of Local Offer website underway in the council 3. Amaze launch of Transitions Information on their Website 4. Integrated Care Board information CYP Mental Health & ND Communications Programme 5. Develop a communication plan to ensure CYP and Parents know how to access the information

Agreed way forward: 1. Signpost parents to current Local Offer – there is funding for resource to update Local Offer and update links to various provision (education, youth employability service, youth pastoral team, ASC – not up to date). 2. Not clear about housing options (invite Housing Representative on SG) Harry Williams to engage on Local Offer website update. 3. Ensure Council’s housing offer is on the various platforms and Local Offer 4. Get information to schools about the various platforms available and the Local Offer. Parents have most contact with schools. Link in with Careers Hub and Family Hub to sign post YP and parents to Local Offer and other platforms 5. Link with Amaze on their website offer. 6. Engage NHS Mental Health Commissioners to understand where the information from the NHS is for YP and parents to access about available support in their transitions journey.

4. Engagement, Consultation & Coproduction: Ensure the models of good practice in place with SEND Services are adopted across all five care pathways. |

· Discussing with PaCC and Amaze about leading on pieces of work on the delivery plan and what a good Transition experience feels like · Continue to engage with the schools to manage expectations and enable conversations with parents to be less confrontational
Draft Actions: 1. Continued engagement with PaCC and Amaze on the development of the delivery plan 2. Engagement with the LEAG (Lived Experience Advisory Group) via Possability People for CYP with mental Health Needs 3. Continued engagement with schools and colleges on the new processes

5. Transitional Safeguarding: Ensure the models of good practice in place elsewhere are adopted locally as well as learning from SARs and Inquests etc. |
Themes:
· Take the relevant learning and key issues from the SAR Oliver action plan 2025 · Consider safeguarding within the framework of risk management, via the AVRM (Adolescent Vulnerable Risk Meeting) in Children’s Services and MARMM (Multi Agency Risk Management Meeting) in Adult Services · Continue to research and learn from other best practice examples nationally – i.e. Birmingham City Council
Draft Actions: 1. Develop a local Transitional Safeguarding Protocol – to be adopted locally and co-opted into the Pan - Sussex Safeguarding Adults Policy and Procedures

Agreed Way Forward: 1. For the Heads of Safeguarding for Children’s and Adult Services to continue to collaborate on developing the protocol 2. For the protocol to be developed with other delivery partners i.e. the NHS etc.
